Curious Old Deep Relief Portrait Carving

Photos don't really do justice to the this carving, which comes off looking a bit rougher and more rustic than it really is, which is quite beautifully done.  It is a curious one though! What to make of this figure, who appears to have a second set of eye balls, up on his eyelids? I would think he is likely meant to be a priest or monk, with skullcap on his head, deeply carved hood open over one should, and neatly buttoned collar at his neck--so then II'd guess those extra eyes are a window to the divine? (Seems not a bad thing to have an extra pair of all seeing eyes around!) 

5 3/4" x 5 3/4" x 1" deep. It appears to have quite a bit of age, but I'm really not sure of its origins. Very good condition, with a nice patina, and small metal hanger on the back. The wood is darker and richer than details portray; overall images are more accurate.
